Illinois School Code (§ 105 ILCS 5/27-8.1. Health examinations and immunizations) requires that students be examined by a physician and verify they have received appropriate immunizations according to a schedule established by the legislature and supported by the Illinois Department of Public Health.

Students are to have complete physical examinations upon entry to the following grades:
  1. Entering kindergarten (or the first grade to begin school
  2. Entering the sixth grade (new requirement for the 2008/2009 school year based on changes in State legislation)
  3. Entering ninth grade; and
  4. Enrolling in an Illinois school, (if the student is transferring into Illinois from out-of-state or out-of-country), regardless of the student’s grade level.
Examination results (including a health history for parents to complete and sign) must be submitted before October 15 of the new school term. Please make sure the immunizations and physical forms are completed and returned by October 15, 2008. The physical exam must have occurred within the last twelve months prior to the first day of school (August 20th is this year’s first day), and the physical must be recorded on the designated State of Illinois form.
